Nokomis Healthy Seniors offers these services and programs to the community:

• Nursing Case Management:  Registered nurses provide one-to-one case management services as a collaborative partner in the care of clients and perform home visits to home-bound seniors.
• Volunteer Services: Volunteers provide direct help, reduce isolation and connect seniors to the community through services such as friendly visiting, transportation services and grocery shopping.  Volunteers also provide little extras like un-sticking a screen door or delivering a gift during the holiday season, and have always been a strength of the Nokomis Healthy Seniors program.
• Referral and Case Management:  Seniors can call the NHS Senior Helpline to ask questions and receive information about services to help them maintain their independence. NHS staff can then refer seniors to community resources, NHS volunteer services and/or NHS nurse case management services.
•  “Nurse is In” Community Health Outreach Clinics:  The “Nurse is In” drop-in clinic is open four hours, once a week for senior clients to access nursing staff for preventive, general healthcare checks (blood pressures, medical questions, foot care, etc.).  NHS also operates monthly clinics at three locations in the community, and all clinics serve as a key social outlet for many seniors in the community. 
• Health Education Programs: Monthly health education programs are led by staff at a neighborhood church and are open to any member of the community.
• Exercise:  Weekly exercise classes are held on-site at Nokomis Healthy Seniors and are tailored to each senior’s ability level.
