Nokomis Healthy Seniors

Nokomis Healthy Seniors is a Living at Home/Block Nurse Program serving the greater Nokomis area of Minneapolis. We help neighborhood seniors to continue living safely and independently in their own homes. We assist seniors in key areas such as providing programs and activities, referral to community resources, and coordinating services for home care.
